I had an interesting experience recently when a patient came to see me. From their body language and behaviour it was immediately apparent that they were anxious, worried, and kinesiophobic. A very brief summary of their journey: they had gone to see a Podiatrist a few months ago with a pain in their foot which was of low severity and irritability but slightly interfering with their ability to run longer distances as comfortably as they’d like. What sounded like a very thorough and appropriate assessment was performed, followed by a discussion of the findings and a plan to move forward. I asked the patient what their understanding of the issue was. They told me they have legs that aren’t the same length, they run “weirdly” and that their arches were “collapsing”. They needed special insoles if they were to have any hope of turning this all around. • I asked them what the diagnosis was; i.e. what information they had regarding the actual problem they had attended with. They did not know. Now, this does not mean they were not told this of course. It is a narrative on the patient perspective. They tend to only recall a few points of information, and they are likely to choose those which they feel are most relevant. • This individual had now stopped running completely, and was refraining from all impact activity in the gym. Was their foot pain resolved? No. It was getting a bit worse if anything. • They had gone to see a healthcare professional with one minor issue. They had left with (what they considered) were 3 major issues, and as a result were in more discomfort and were less able to participate in an activity they enjoyed for both the physical and mental benefits it brought. • It’s been forcing me to further reflect on the way I communicate with others, so its been pretty useful in that regard. This isn’t pedantry. Words (and most importantly the way they may be interpreted) matter. Os dados foram examinados qualitativamente usando uma análise de conteúdo: . ➡Fatores físicos: 84,8% (atribuição a causa biomédica), tais como: 35% movimentos ativos; 28,1% posturas; 5,3% esforço repetido; 4,4% alteração biomecânica; 4% comorbidades; 3,3% sedentarismo; 1,8% trabalho; 1,% medicamentos. . ➡Fatores psicossociais: 15,2% (atribuição a causa não biomédica), tais como: 6% estado psicológico; 5% condições climáticas; 2% sono; 1,2% dieta; 1% fadiga. . Estes resultados indicam que os indivíduos consideram os fatores biomédicos como os principais gatilhos da dor lombar, mas alguns reconhecem gatilhos não biomédicos também envolvidos. . O reconhecimento sobre esse ponto de vista (causas da crise) pode ser crucial para ajudar a desenvolver uma estratégias de prevenção na prática profissional, e criar possibilidades de como modificar percepções dessa condição mal adaptativa. . . Esse é o primeiro estudo a investigar as percepções de profissionais como o fisioterapeuta (FT) e terapeuta manual (TM) sobre qual seria uma “postura” segura para levantar algo do chão. A coleta de dados foi feita por pesquisa eletrônica, utilizando o questionário Atitudes de dor nas costas (Back-PAQ survey - Darlow et al., 2014) cada um com 5 respostas possíveis. Ela foi projetada para destacar crenças que são consideradas duvidosas para a recuperação de um episódio de dor nas costas, por exemplo, crenças sobre a vulnerabilidade das costas, relação entre dor e lesão, prognóstico, atividade e dor, influências psicológicas na dor nas costas, dentre outras coisas. Em seguida, o participante teria que escolher uma das 4 posições (acima👆) baseada no seguinte questionamento: “Considerando a carga na caixa, é um peso que o sujeito acha pesado, mas é possível levantar. Qual posição você considera ser a mais segura?", e uma caixa de comentários foi fornecida perguntando: "Por que você escolheu essa técnica?" . Os resultados deste estudo mostrou que a maioria dos FT (76%) e TM (91%) acreditam que levantar com as costas reta é mais seguro do que com as costas curvadas (arredondada). A justificativa de escolha para posição reta (descrição biomecânica): 51% ➡“coluna neutra”, “costas retas”, “boa postura”; 20% ➡“postura da cabeça”. Por outro lado, quem escolheu a posição arredondada usou termos mais globais para justificar sua escolha: 28% ➡“eficiente”, 32% ➡“relaxado ou confortável”. . Em um dado momento, já foi até especulado que as costas tem um número limitado de ciclos de flexão (Callaghan e McGill, 2001) e que a flexão deve ser evitada para poupar a coluna. No entanto, a transferência desses estudos e suas suposições foram desafiadas pela modelagem in-vivo, que não conseguiu mostra que a postura das costas é um dos principais determinantes da carga espinhal (Dreischarf et al., 2016). . Belief plays an important part in making any change.⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Take pills to fix your headache. You’re in the supermarket and see the cheap own brand paracetamol alongside a branded packet in its shiny packaging, which is much more expensive. If you believe the more expensive branded pills will work better, even though they have the same ingredients as the supermarket own label, then they may well do.⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Eh, how can that be? It’s because of the placebo effect – i.e. the level of your belief in a treatment effectively affects its effectiveness. In a similar way if you give someone a sugar pill and they believe it’s paracetamol it may well work.⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀ You may well have used the placebo effect as a parent. Numenorean - Adore Post-black metalové kapely mají jednu schopnost, která se mimo tento žánr zas tak nevidí - a nebo jsem na ní ještě nenarazila. A tou je skutečnost, že v sobě mají něco, co dokáže člověka totálně a absolutně pohltit, uzavřít do bubliny samoty i v davu. Každý máme svůj žánr - a u mě je to očividně právě post-black metal. Co se textové stránky týče, jak je mým zvykem, ještě jsem se na ni nevrhla. Nicméně hudebně- Numenorean v sobě mají temnotu, která dýchá z každé noty. V dlouhých nahrávkách střídají klidné temné kytarové melodické pasáže s krásnou vyvážeností střídají úderné bicí a zpěv, sakra, z každého jednoho slova je cítit bouře emocí, nenechají posluchače stejného jako před poslechem. Každá jednotlivá písnička je jiná než ostatní, přesto jako by je něco spojovalo. Mají duši. Jsou zkrátka věci, které se nedají napsat slovy - musí být cítit. A Numenorean jako by byli stejným případem. Zkrátka nemůžu říct nic jiného než - prostě si to album poslechněte. Možná jsem ho zatím slyšela jen jednou, ale to v budoucnu hodlám rozhodně ještě napravit. A i na první poslech, už mám vytipované stopy, které mě oslovily nejvíce. Essa basicamente foi a pergunta desse estudo transversal (realizado em uma cidade australiana = Uma das limitações do estudo) com 67 pessoas sem dor (sem dor ou dor média ≤3 / 10 por menos de uma semana nos últimos 12 meses), que contemplou uma pesquisa on-line contendo dados demográficos e medidas autorreferidas de vários pontos, porém vamos nos atentar aqui a um tópico da pesquisa que era presencial: Teste implícito de associação (Implicit Association Test) que avalia o individuo sobre sentimentos ou opiniões inconscientes sobre algum assunto, usado na psicologia da saúde e pesquisa em dor. . . O participante olhava 12 imagens de vista lateral (aleatoriamente) de pessoas nessas 2 posturas: A ("round-back posture" = coluna curva/arredondada)  e B ("straight-back posture" = coluna reta/neutra), e deveria classifica-la em “Perigo” ou “Segura”, forneceram também uma descrição de sua compreensão do que seria um “levantamento seguro”, baseado no resultado do teste os participantes associaram a posição A como representando “perigo”. . . Tendemos a tomar decisões sobre comportamentos de uma tarefa adotada no cotidiano que podem ocorrer como parte de um processo “dual”, no qual é explícito (deliberado, analítico) e implícitos (automáticos), que estão envolvidos na avaliação de risco versus benefício associado a tarefa. Suas influências no comportamento da tarefa pode variar dependo do contexto, tempo e motivação. (de Jong JR, 2005; Leeuw M,2007; Goubert L,2003). . Até o momento existe uma falta de evidência in vivo que apóia a noção de que usar a "straight-back" em preferência a um "round-back" pode reduzir o risco de dor lombar (Wai EK,,2010; Kingma I,2010). Estudos biomecânicos encontraram não haver diferenças significativas nas cargas e compressão da coluna vertebral entre as duas posturas (Dreischarf M,2016; Kwon BK,2011). Last Day of #AWeekOfResearch goes to: The Placebo Effect🧠⁣ •⁣ I really just wanted to highlight how powerful the mind is in healing (or hurting #nocebo) the body.⁣ •⁣ Presented are 3 research papers. The first touches on placebo pain relievers and their comparable effect to actual analgesics. While placebo analgesia is related to the release of endogenous opioids (enkephalins and endorphins primarily produced in the brain) this study also showed placebo-induced dopamine release leading to motor improvement in Parkinson's disease. ⁣ •⁣ In the 2nd study we look at placebo in athletics. Research in athletics suggests that much of the impact of such aids may be due to expectancy-the belief that the substance will aid performance. This study used sodium bicarbonate and placebo additives with athletes completing a series of 1,000m time trials. Results showed that believing one had taken the placebo resulted in times almost as fast as those associated with consuming the drug itself. In contrast, taking the drug without knowledge yielded no significant difference.⁣ •⁣ And just to really drive the power of placebo home, I’ve included a study on placebo surgeries. Yes, that’s right. “Placebo surgery” is a thing, and research shows that it works. In this systematic review, 39 out of 53 (74%) showed improvement in the placebo treatment and in 27 (51%) of the trials, the placebo did not differ from the surgery.⁣ •⁣ As much as I’d like to write a 10 page paper on all of this, IG character count has its limits lol. Last night I attended/hosted Bill Adamson's seminar on language in practice in behalf of Beyond Mechanical Pain. ~ Bill took us through a history of pain and how posture has it's roots in classism. ~ He then linked the idea of language with how we describe pain. ~ Finally, he explored what effect language might have in practice and how we can optimise it to help patients feel empowered and recover. ~ It's funny how we got where we are today, based on many incorrect ideas and assumptipns! Let’s talk about HEXING . I know it might sound a bit dramatic, but hear me out. Hexing is a very real thing. And it affects all of us. Especially those who are trying to heal chronic illness. . Most of us are familiar with the placebo effect. And on the flipside of that is something called the nocebo affect. This states that when we are told something negative, or that something will do something bad to us, that becomes the most likely outcome. Hexing is simply an extension of this truth. . Your friend tells you that the new supplement you just started will make you break out in a rash. The guy at the grocery store makes a comment about how tomatoes can cause panic attacks. One of your doctors tells you that you only have a 10% chance of ever getting better. These are all examples of hexing, and the last one is actually known as “medical hexing.” (I highly recommend you google this- the stories will blow your mind) . We all like to think that we’re not as affected by people’s words as we actually are. But the truth is, those little comments, all the advice, those diagnoses and prognoses, sink deep into our psyche. And they can dramatically affect our health outcomes. . While we can never control what other people are going to say to us, I think it’s important to have a conversation around hexing. It’s important to be aware about how other people’s words and opinions will affect us. Guten Morgen ☀️ Den Placebo Effekt kennt ihr wahrscheinlich alle oder? Und den Nocebo Effekt? Der Nocebo Effekt ist quasi das Gegenteil des Placebos. 🌼 In Studien wurde rausgefunden dass bei einer Verabreichung von Medikamenten ohne "Wirkung", wo aber viele negative Nebenwirkungen in Aussicht gestellt wurden, diese tatsächlich auftraten. Eine Tbl wurde genommen, beispielsweise gepresster Süßstoff, und es hieß: "aber vorsichtig, viele bekommen davon Übelkeit, bis hin zu Erbrechen". Ihr glaubt gar nicht für wieviele Menschen die Übelkeit tatsächlich da war!🤒 Damit will ich sagen, achtet auf das was ihr eurem Unterbewusstsein an Gedanken sendet. لدىٰ #الصّيادلة مُصطلح مشهور يُطلق عليهِ #Placebo " #بلاسيبو " و هو من الأدوية الّتي لا تملُك في تركيبتها مادّة فعّالة . الموضوع بدأ فى الحرب العالمية الثّانيّة حيثُ تعرّضت الجيوش للشُح بمادة المورفين ، أوهمت إحدىٰ المُمرّضات أحد الجرحة بحقنه بالمورفين لكنّها ملأت الحُقنة بمحلول ملحي على أنّه مورفين و أدّى ذلك إلى تسكين ألم الجريح . الموضوع تطوّر بعد ذلك بشكل أكبر ، حيثّ عمِد العُلماء عند تصنيع دواء جديد ، إلى تجربتهِ على مجموعتين من الناس ، مجموعة تأخذ الدّواء و المجموعة الثانية تأخذ البلاسيبو ، كي يتأكّدوا من الفائدة العلاجيّة للدواء . على الجانب الآخر من التأّثير النفسي يوجد هُناك مُصطلح ليس مشهور كثيرًا " #Nocebo " #نوسيبو " و هو النقيض للـ بلاسيبو . هنا الأشخاص يشعرون بألم وهمي نتيجةً لاعتقادهم أنّهم سيتألّمون . أحضروا مجموعة من النّاس و عرّضوهم لجهاز يُصدر أشعّة ليزر لقياس كثافة العظام و أقنعوا هؤلاء الأشخاص أنّ الضوء لهُ تأثير حارق و يجب أن يتحمّلوا الألم لأطول فترة مُمكنة ، و إن لم يستطيعوا الاستمرار سيطلبون من الطبيب إيقاف الجهاز . الّذي حدث أثناء التّجربة أنّ الأشخاص كانوا يتألّمون بشدّة لدرجة أنّ بعضهم أخذ بالبُكاء وطلب إيقاف الجهاز بعد ثواني من تشغيله .

Tänään kun heräsin, päässä soi Stam1nan Pirunpaskaa. Piti oikein laittaa kyseinen biisi soimaan kun tukkaa suoristin ja aamukahvia join! Töihin laitoin Stam1nan SLK-hupparin. Se on vähän liian kireä nykyisin. Mietin, että liian pitkä aika siitä kun oon viimeksi nähnyt bändin livenä. Nää kuvat on vuodelta 2013 muistaakseni. Harjavallasta, Kebab Danger -kiertueelta. Se oli aivan älytön keikka, bändi alkoi soittaa todella myöhään ja jengin keskimääräinen promillemäärä liikkui varmasti kolmen yläpuolella. Jassu ja Ida oli mun mukana. Hauskoja muistoja! Ja onhan nää kuvat aivan timanttisia, vai eikö muka oo?! 😂

Elle est très profonde et assez complexe vu de loin. Une fois plongé à l’intérieur il est surtout question de logique et de probabilité. . 🔸Après cette découverte j’ai commencé à comprendre que l’importance que j’accorde à limiter l’effet nocebo est encore plus important que je l’avais imaginé. 🔸𝗕𝗮𝗱 𝗜𝘀 𝗠𝗼𝗿𝗲 𝗣𝗼𝘄𝗲𝗿𝗳𝘂𝗹 𝘁𝗵𝗮𝗻 𝗚𝗼𝗼𝗱: 𝗧𝗵𝗲 𝗡𝗼𝗰𝗲𝗯𝗼 𝗥𝗲𝘀𝗽𝗼𝗻𝘀𝗲 𝗶𝗻 𝗠𝗲𝗱𝗶𝗰𝗮𝗹 𝗖𝗼𝗻𝘀𝘂𝗹𝘁𝗮𝘁𝗶𝗼𝗻𝘀🔸 . ▪️𝗡𝗼𝗰𝗲𝗯𝗼 is a negative effect on health from an intervention . 🔹Two factors affect our ways of mediating the placebo/nocebo responses; Conditioning & expectations. What has this patient already been told; what do they believe will happen; what do they expect from your consultation etc. . 🔹Two key factors to understand nocebo are the concepts of validation and invalidation . ▪️𝙑𝙖𝙡𝙞𝙙𝙖𝙩𝙞𝙤𝙣 is the act of communicating acceptance and understanding to another person . ▪️𝙄𝙣𝙫𝙖𝙡𝙞𝙙𝙖𝙩𝙞𝙤𝙣 is the opposite, the act of communicating non-understanding and non-acceptance to another person . 🔹Oftentimes, we focus on the impact of the positives such as empathy & compassion (which are certainly essential), but it is also essential to focus on how NOT to invalidate people as the negative effects of invalidation & a nocebo response can have strong negative effects on patients . 🔹Invalidation can make patients feel dismissed & disbelieved, hopeless & angry or to avoid certain treatments/practitioners due to negative experiences/expectations . 🔹Invalidation can be delivered unintentionally; i.e if a patient is in a lot of pain and they bring in an MRI that only shows a small rotator cuff tear and you say, “it’s okay there is tons of evidence and tons of people that have small rotator cuff tears but zero pain” . ▫️Certainly well intentioned & evidence-based, but validating how the patient may feel at that point in time first and then facilitating their re-education of their beliefs and expectations concerning their MRI . 🔺It is very easy to invalidate or fail to validate someone unintentionally; our words may be misperceived by the patient, especially if our education opposes their current conditioning, expectations and/or views; we must continue to do our best to not cause unnecessary and unintentional harm by being selective of the wording we use with patients
